Analysis of Mental State and Depression in Patients with Systemic Lupus Erythematosus Monitored in a Rheumatology Service of Manaus

Abstract

Introduction: Systemic lupus erythematosus (SLE) is a multisystemic disease that can cause neuropsychiatric impairments involving the central nervous system, among them cognitive disorders and depression are most common. Objective: To check the prevalence of cognitive impairment and depression in SLE patients and its association with the time and activity of the disease, cumulative corticosteroid dose, treatment adherence and neurological involvement Methods: Cross-sectional cohort study on 70 SLE patients followed up at a rheumatology service. We evaluated patients of both sexes, aged between 18 and 55 years old, who fulfilled the American College of Rheumatology (ACR) criteria. A chart review was performed to collect data and a clinical examination was carried to measure the Mexican Erythematosus Systemic Lupus Disease Activity Index (MexSLEDAI). The tests used to assess the cognitive impairment and depression were the Mini Mental State Examination (MMSE) and the Beck Depression Inventory. Results: It was verified that the prevalence of depression is of 50% and cognitive impairment is of 20%, besides there is also a statistically significant relationship between them (p = 0.03). The other variables of the disease were not significant when related to depression and cognitive impairment. Conclusion: A high prevalence of cognitive impairment and depression was verified in patients with SLE and an association between them. The MMSE and the Beck Depression Inventory were effective as screening tests to detect these events and their use should be encouraged in order to improve the quality of life of those patients.
