Renal malacoplakia is a chronic granulomatous disease, poorly defined etiology, which can affect the vast majority of organic systems, however, the largest local incidence of this disease is related to the human genitourinary tract. Currently, its cause, has been linked to immune system dysfunction and phagocytic and persistent infections with gram-negative organisms represented here mainly by Escherichia coli. We report the case of kidney malacoplakia a patient underwent total nephrectomy right after suspicion of renal neoplasia, however, with pathognomonic findings of kidney malacoplakia blades after revision arising from kidney removed piece is 02 years likely due to local recurrence.
