The incidence of cardiovascular complications associated with COVID-19 in Diabetes Mellitus patients: a systematic review

Abstract

INTRODUCTION: Diabetes mellitus (DM) is a group of metabolic disorders characterized by serum hyperglycemia. Diabetics more easily develop serious complications, especially cardiovascular, infectious and immunological complications. COVID-19 is a disease caused by SARS-CoV-2 which, in addition to pneumonia, can cause damage to other organs, causing gastrointestinal, neurological and cardiovascular sequelae. DM carriers tend to have a greater inflammatory condition than non-carriers and, therefore, are prone to worsening respiratory syndromes resulting from COVID-19 and cardiac complications. OBJECTIVES: To review existing literature on the relationship between COVID-19 and cardiovascular complications in DM. METHODOLOGY: A systematic review was carried out including publications from the identification of Sars-Cov-2 until June 2021 in the MEDLINE/VIA PUBMED, SCOPUS and Web of Science databases, using PRISMA rigor. RESULTS: The searches resulted in the inclusion of 13 eligible articles. It was possible to determine the incidence of cardiovascular complications in diabetic patients with COVID-19, in addition to verifying new cases of post-infection DM and observing higher mortality in DM patients. DISCUSSION: There are few studies showing the relationship between COVID-19 and the development or exacerbation of cardiovascular complications in DM patients. One of the most common complications was acute cardiac injury, while the most common comorbidities were hypertension, CVD and chronic kidney disease. CONCLUSION: There is a relationship between COVID-19, CVD and DM, with the combination of the three being the greatest lethality factor. More studies are needed to investigate the medium and long-term outcomes of endothelial damage and the inflammatory environment present in diabetic patients.
